the Wyoming Young Adult Survey!

We need individuals between 18 and 29 to share their opinions about substance

use and other health behaviors. The information collected will help improve and

inform prevention efforts in your community.

By spending 15 minutes of your time answering the survey,

you could win a $25 gift card. At the end of the survey, there

will be a link to enter a drawing for one of twenty-five $25.00

gift cards and one of two $100.00 gift cards to be given away

in January 2023. The drawing link is NOT connected to your

survey responses.

The Wyoming Department of Health (WDH) has contracted with AFMC to conduct

the Wyoming Young Adult Survey. The survey is anonymous and does not

record any personally identifying information.
